A mix-up led to the remains of a UK serviceman killed in Afghanistan being returned in the wrong coffin, the Ministry of Defence has admitted.

How does any human error happen? It could be carelessness or could be that people were given too many other things to do at the same time. Sometimes even with the best will in the world, mistakes occur.
